Community Coaching

Community coaching is a service designed for individuals who need one-to-one support in a variety of community settings in order to build a specific skill or set of skills to address particular barriers that prevent individuals from participating in activities of community engagement. In addition to skill building, this service includes routine and safety supports. Community coaching service shall be covered by the developmental disabilitiy waivers.

Activities

Learning experiences and activities – Yoga, Social Groups, Community Events, Amusement Parks
Recreation – Movies, Bowling, Games
Peer support groups
Shopping
Employment support
Volunteer work – community clean up, or other neighborhood service

Start Our Intake Process Today!

Our intake process includes a comprehensive assessment by a case manager or support coordinator assigned to the individual.

This assessment includes:

psychological evaluation
current physical
list of medications
Supports Intensity Scale (SIS)
Risk Assessment
VIDES
Support Plan (If individual is currently receiving services
Copies of doctor's orders must be obtained before services can begin.

Our intake process is designed to ensure that individuals and there familie receive the best care possible.

Insurance & Payments:

We accept the Development Disabilities Waiver, also known as Community Living Waiver, issued by the local community service board and private pay. The home’s administrators will discuss payment details before admission.
